Required Updated Enrollment Documents

Choose the file below that is required for your child's renewal. Each year your child will need a Health appraisal completed by a physician, new enrollment form, (school age children only) Good health statement. Once you have returned required items you will also have to review your child's emergency card front and back and initial in provided area. All forms must be completed and returned to the Main Office.

All immunizations must be current or have a waiver form given by the Department of Health and Human Services.
